The mechanism of action of zinc to inhibit ion transport as mentioned by you [1] is based on the report published in Jan 2011. [2] This was after our paper was sent for publication. [3] However, the facts mentioned in our paper are based on the literature available that time. We accept the new findings mentioned by you that are supported by the new published literature.

Regarding the efficacy of zinc based on the etiological factor, we would like to clarify that we did not take into consideration the etiological factors but discussed in general the beneficial effects of zinc in diarrhea. Even otherwise clinically it is not always possible to establish the causative organism and treatment cannot be withheld till reports of laboratory investigations are available. Further, WHO has recommended that zinc supplements should be administered in diarrhea from the day of onset of symptoms along with ORS. [4]

We agree to the probable reasons proposed by you as far as the differential effects of zinc in diarrhea are concerned as no published study has investigated the efficacy of zinc in zinc-deficient state.
